Custom HTML Form with Google Sheets

I am looking for a developer to create a custom HTML form that displays 175 questions one by one (at the moment questions are in google forms, can be connected to HTML), with the option to add 5 more Quizzes. The form should be designed to save responses directly into a Google Sheets document using a Google Apps Script Web App. Each user should access the form through a unique link, and the form can include a timer feature.

Key Requirements:
- Develop a custom HTML form with 175 questions, (5-6pcs in one) using checkboxes for input.
- Implement functionality to save answers in Google Sheets via Google Apps Script.
- Ensure each user accesses the form through a unique link.
- Include a time countdown , one time mail use mandatory.
- Design the form with a simple and clean aesthetic.
