Google forms based system for factory production

Project Description:

I am seeking a skilled developer to create a Google forms based system for our factory production. The system should have the following features:

a) Create forms based on detailed instructions I attach.
b) Record everything done by our workers (submitted by our team leaders to these google docs).
c) Be able to read from an auxiliary worksheet so workers choose among certain options.
d) Perform certain calculations (not complex).

At this stage that is all. However, we aim to make automatized reports, in the future once all this has been successfully achieved and implemented at our factory.

Finally, we are absolutely open to suggestions since we are not specialists in this area!!

I attach the word file that includes all details. Besides that, I include two excel files: input file (all information that will be needed so people that complet google forms can chose from certain options) and output file (which is the file that will contain all data entered by these people).
