Google Form-to-Google Doc Automation

Project: Automated Google Form → Google Doc Intake Workflow
I am seeking a streamlined, automated workflow for intake paperwork. I will send you a Google Form that collects personal and clinical intake information. Immediately after each submission, I want it to generate a fully populated Google Doc using a template I will provide.

Some questions will be yes/no, others short answer, extended response, or checkboxes. All responses must map cleanly into designated placeholders within the document template.

Project Requirements
1. Google Form Setup
• I will send you a Google Form that gathers:
o Standard personal information (name, DOB, pronouns, address, contact details)
o Reason for referral
o Relevant intake questions specific to my psychology practice
• Mixture of field types: yes/no, short answer, long answer, checkboxes.

2. Document Template (Word or Google Docs)
• Use the template I supply.
• Insert merge placeholders in all required sections.
• Every form response must flow correctly and neatly into the designated locations.

3. Automation Workflow
• Use Google Apps Script to:
o Trigger automatically upon form submission.
o Create a new fully populated document using the template.
o Save the completed document in a specified Google Drive folder.
o Name the file using a defined format (e.g., “Intake – {{ClientName}} – {{Date}}”).
o Optionally email a copy to an address I specify.

4. Deliverables
• Fully configured Google Form.
• Google Doc template wired to the form fields.
• Automation script or add-on (Apps Script preferred) to generate, store, and optionally email documents.
• A brief setup/editing guide so I can:
o Modify questions
o Add/remove fields
o Update placeholders without breaking the automation.

Ideal Experience
• Prior work with Google Apps Script, Workspace add-ons, document automation, or mail-merge style workflows.
• Experience with professional/clinical documentation is a bonus.
If you have previous experience with Google Apps Script automation or mail-merge style document generation, this should be a quick, clean project.
