Convert Fillable PDF to Google Doc

We are seeking a skilled freelancer to transform a fillable PDF into a fillable Google Doc while maintaining the original formatting. The ideal candidate will have experience in document conversion and a keen eye for detail to ensure the Google Doc matches the PDF's layout. I wouldl ike this done by Monday Novermber 20.

Ensure fillable fields and check boxes are functional
This is a legal document, so maintaining identical appearance to the pdf is important.
The google doc needs to function the same as the pdf. I will be using the google doc for autofilling from a script.
The "Division" box does not need a drop down menu (which the pdf has); it only needs a line to enter text
If a sentence ends with a colon ":", it needs lines that can be filled in after it or below it, e.g., "Accordingly, the Court attributes income of $ per based on:" would need to be able to accept text below that sentence"
