Resolve website HTML error

Website seems to have some errors that preventing my html form from being loaded..

https://www.wresidencesmarinaview.com/about >> this is a subpage that is loading on main homepage.. it works when u view it in "https://www.wresidencesmarinaview.com/about ".. but when you view in homepage "https://www.wresidencesmarinaview.com/", the error message appear..
This project just need to solve this error and make sure the html webform appear.
