Chrome extension that scans PDF417 style codes

I’m looking to build a chrome extension that will activate the camera on a cellphone and/or a computer which will scan a PDF417 style barcode from a state-issued ID card. The extension will recognize the current URL’s preloaded mapping and then paste certain contents from the code (first name, last name, address, etc.) to certain text fields based on their form ID. The idea is for the user to scan an ID card and have that card’s information populate on whatever form they are trying to fill out.
For example, if the Chrome extension scanned the ID card on the PDF417 barcode guide I’ve attached, the “First Name” text box on the attached screenshot with the ID of “79” would be filled with “JOHN”
I would need to have the ability manually map each individual barcode content to their corresponding text fields. Each text field ID will differ from URL to URL.
