patient referral letters
Budget: $750 – $1,500 AUD
A eneral practitioner works at two different locations in Queensland who both have their own practise management software. He is specifically looking for a cloud based solution for generating letter referrals to specialists based on templates.
He will maintain his patient database of details like patient name, date of birth, medicare number, etc. which can be a google spreadsheet
He will need two template letters with different letterheads for the two locations he practises out of. The template has the letterhead at the top, then the date, then the address where the letter is to be sent, a salutation, a reference line based on the patient details, and headings based on which inputs were given for the situation, such as "Hx of Present Illness", "Last Review and Referral Details", "Current Medications", "Allergies", "Surgical / Past History / Procedures" and "family history". He should be presented with a set of form inputs like Clinic Location (used to determine which letterhead to use), and fields he can enter or leave blank like "History of Present Illness", "illicit Drug Use History", then vitals like blood pressure (BP), Pulse, Weight, Respiration, Temperature, Skin Examination, Head and Neck examination. There will need to be rules as to what gets put into the referral letter based on what inputs are given and which are blank. Then he will need some text form inputs for "Chest Examination" "Head Examination" "Abdomen Examination", "AAO Examination", "Psychiatric Examination" , "Musculoskeletal Examination", "Assessment and Plan" and "Next Review Due Date". Those fields of course will need to be configurable. He does NOT need appointment management or anything like that. He will save the referral documents produced with the practise management record software which is different at both locations. Could you please advise on costs and your recommendations in this regard?
