Google App Script for Form Responses

I'm looking for a Google App Script that takes responses from a Google Form and saves them to a Google Doc. The Google Doc should be formatted text with headings, specifically using question titles as the headings. Each response should be saved on a separate page. An excel sheet with the headers is attached.
