Google apps integration for membership sheet with mailmerge

Hello, I need a membership spreadsheet, simple name/address/email/ID etc, that can keep track of club members, notify expired members and generate a specific ID card periodically for all, some, or one-off members. Would also like to email ID cards like a mailMerge. We have a JotForm that accepts new members and I'd like to pull that data into the membership sheet, or get rid of jotform and just do a membership application on google. Input form is also possible.

I have a rough partial solution now in excel/word but want to transition and make more solid so others can use it on google docs.
