App script for a Google Form

First would it be possible to assign a unique number in column A of the linked spreadsheet for each form entry. This could be called, "PO Request #".
Next, we would like when the form is submitted, that an email automatically go to the department head that Asks the department head, "Is the Purchase Request below Approved or Not approved"? and below the question has a summary of the Form contents and includes the attachment required by the form. Ideally the subject line would be, "PO Request [insert unique #] " and I would like cc'd on the email to the department head the requestor's email address and ap(at)9meters.com.
For testing you can use my email address bmclaughlin(at)9meters.com as the department head, once the app is ready to go I will replace that with the email address of the actual department head.
