Google Ads Conversion Tracking Setup

My HTML website redirects visitors to a dedicated thank-you page after each form submission, yet Google Ads still shows zero conversions. I already use the global site tag (gtag.js) across the site, so the base code is present; the issue is with the conversion action itself—either the tag isn’t firing or it’s firing incorrectly.

Here’s what I need from you:
• Audit my current gtag.js implementation and locate any conflicts or missing snippets.
• Create and install the proper conversion tag tied to “Form submission” in my Google Ads account, making sure it triggers only on the thank-you page.
• Test the setup in real time (Google Tag Assistant / Preview mode) and verify that conversions register inside Ads, not just in the browser console.
• Provide a brief hand-off note explaining which lines you added or adjusted so I can maintain the code myself later.
