Convert a Power Point Quiz into an interactive Game + A graphic designer to design it professionally

Hi, I want someone who has experience in developing simple games, like quizzes, who wants to be a millionaire. What I want is that there will be two teams and names of both should show on sides and each team answer one question and if correct add to their points which shown below and also how many questions they have answered. You can use HMTL5 or any technology as Flash is deprecated now. If you have developed something like this that you can show then it would be great. I have a web hosting and name and that can use to upload that webpages/game if that is a better option. Later I also need a graphic designer to professionally design it.
