Cloud based application. Hosted on Azure. For healthcare setting. Patients scans QR code on phone. Accesses a form to fill in symptom data. Data stored on SQL database. Once pt is onboarded the app is completely automated. Sends out regular email reminders to patients at pre determined points to collect further data. All data collated and processed to produce an out put for hospitals and clinicians to determine treatment effectiveness and complication rates. Portal will have both patient facing and healthcare provider facing elements. Must be highly customisable in order change outputs. Must be NHS digital accredited.
