AI-Native Healthcare App Migration

Job ID: 39860515

Budget: $10,000 – $20,000 USD

Project Overview
We need an experienced full-stack web developer to lead a phased migration of our MVP healthcare app, D-Path (dpath.help), from a basic Hostinger Horizons-generated setup to a robust, AI-native stack. D-Path optimizes hospital patient discharges by ingesting EHR data, predicting choke points, and enabling seamless handoffs to post-acute facilities—aiming for nationwide scale (100M+ patients). The migration must be no-downtime, HIPAA-compliant, and focused on extensibility for real-time insights and automations. Budget: $10,000 - $20,000 USD; Timeline: 4-6 weeks part-time.
Current State

Front-end: React components generated via Hostinger Horizons (drag-and-drop AI prompts).
Backend/Logic: Node.js for APIs and app flows.
Database/Hosting: Supabase (PostgreSQL DB, auth, cloud APIs)—simple but unscalable for high-volume EHR data.
Challenges: Limited full-stack awareness in prompts; no shadow DB for unstructured data; manual integrations for payer checks/handoffs.

Desired Future State & Key Features
Migrate to a modular architecture (see attached diagram for visual overview):

Front-End/UI: Embed open-source bolt.diy chat interface (React-based) for prompt-driven "vibe coding"—replicates Hostinger's chat+preview but with full-stack gen (e.g., prompt → React UI + Node API + DB query). Integrate Grok 4 Pro API (via LangChain.js) for non-dev code building; support self-service API key insertion (encrypted, no dev help).
Hosting/Deploy: Netlify for serverless CI/CD, global CDN, and edge functions—enables Dev (dpath.help QA) → Prod (dpath.co live) promotions with zero downtime.
Data Layer: Keep Supabase as primary; add MongoDB Atlas shadow DB for schemaless EHR exploration (e.g., discharge notes). Use Debezium/Kafka for real-time CDC sync and Prisma ORM for unified queries—allows future flip to Mongo without rework.
Integrations/Automations:

RPA Bot Layer: Trigger bots (UiPath-ready) on discharge events to scrape/write notes in Epic EHR—feeds insights to DBs for 20-30% faster flows.
Receiving Sites: FHIR/REST APIs via PointClickCare Gateway for handoffs to SNFs, LTCs, rehab, hospice (OAuth-secure multi-site sync).
Eligibility Check: Real-time 270/271 EDI API via Change Healthcare—triggers on patient add to verify coverage.
Prior Auth: FHIR CRD/DTR/PAS API via Availity (170+ payers)—auto-submits pre-approvals on discharge candidates to cut denial delays.

Extensions: Next.js for SSR/performance; Redis caching; GitHub Actions/Sentry for DevOps; HashiCorp Vault for secrets mgmt (HIPAA-compliant keys).

Scope of Work

Export/refactor current React/Node/Supabase code; set up bolt.diy embed with Grok integration.
Implement Netlify deploys and Dev/Prod environments.
Configure Mongo shadow sync (Debezium/Kafka/Prisma).
Build/test integrations (RPA, PointClickCare, Change/Availity APIs)—focus on real-time triggers and error handling.
Document setup, including HIPAA notes; provide 2 weeks post-launch support.
Weekly check-ins; use GitHub for collab.

Required Skills/Experience

Full-stack: React, Node.js, Next.js, Netlify (or similar serverless).
Data: Supabase/Postgres, MongoDB, Prisma, CDC tools (Debezium/Kafka).
Integrations: FHIR/REST APIs, RPA (UiPath), healthcare payers (Availity/Change Healthcare).
AI: LLM APIs (Grok/LangChain.js), prompt engineering.
Bonus: Healthcare domain (EHR like Epic, HIPAA compliance).
Portfolio: Similar migrations or healthcare apps; 4.8+ rating.

The more detailed your proposal (e.g., approach to RPA triggers or cost breakdown), the better—let's discuss on call!
