Build a simple Webapp to create a pdf report

Currently we create few reports using MS Word Template. We now want a Web App to create the same.
The report has fields like Radio buttons, checkboxes and Free Text. Few sample fields:

Name
Gender
Phone
Email
Address
Free Text#1
Free Text#2, ...etc.

Required UI/UX:
1. Login Screen
2. Dashboard to choose either “Create Report” or “View Report”
3. For “Create Report” display a new form. And Save in a database table.
4. For “View Report” read the fields from database table and display.
5. Give a button to Email the report as a pdf file.

Note:
In future we plan to integrate this Webapp to a Mobile App
The database tables should be flexible to add new fields in future
