Interactive HTML Report Development
Budget: $750 – $1,500 AUD
I would like a HTML report to be created that has specific functions to allow a user to complete off line. It needs to look good as it will be printed and give as a product. It is for therapists to use and recrod functional information about their patients. I have already sarted and have basic prototypes using AI but the functions always full down. See attached.
Some basic prompts:
There should be a footer that includes Rivergum Allied Therapies – FCA, the name of the client and then page number.
All buttons should be icons
Any button, toggle or instruction should not appear when printed.
There should be a print to pdf button, clear and save as button. There should also be an autosave function in built.

Each subsection needs to have a box that when ticked populates a line to be completed in a recommendations section at the bottom of the report. Each recommendation line has to have fields to complete for Goal, Effective and beneficial, Related to disability, Related to goals, value for money, Funding (This will need to have lines that can be populated with headings that include Funding area, Funding type, hours/budget).
Each subsection below also needs to have a rating scale for level of independence using the FIM scale 1-7, COPM performance and COPM satisfaction. These scores need to be produced into graph format at the end of the report above the recommendations section.
Each subsection also needs to have a button that when clicked greys out the section so it does not appear when printed.
Each subsection needs to have a function where a supervisor comments box can be toggled to appear, the font be in red. This should not show up when printed.
