GoHighLevel Survey CSS & JavaScript Styling

I need an experienced front-end developer to restyle my GoHighLevel survey to match a modern, clean design example I will provide. The final deliverable will be a block of CSS and a snippet of JavaScript that I can paste directly into the platform.

Key Requirements:

CSS Styling:

Replicate the visual style of the target website (fonts, colors, spacing).

Convert all radio buttons and checkboxes into full-width, clickable "cards" with hover and selected states.

Ensure image-based options are styled correctly (vertically aligned with smaller images).

The design must be fully responsive for mobile and desktop.

JavaScript Functionality:

Implement a Metric/Imperial unit switcher for height and weight inputs. The script must handle the on-page conversion logic.

Future-Proofing:

The code must be clean, efficient, and well-commented.

Use stable CSS selectors (like data-q attributes) where possible to reduce the risk of breaking when GoHighLevel updates its platform.

Use modern layout techniques like Flexbox or Grid.
