Bootstrap Conversion & Dynamic Form

I have an existing website that needs to be rebuilt with the Bootstrap framework, and I’d like a small enhancement added at the same time.

Bootstrap conversion
– Migrate the current HTML/CSS into clean, component-based Bootstrap markup.
– I’m flexible on the version; feel free to use either Bootstrap 4 or 5 as long as the final result is fully responsive, lightweight, and cross-browser friendly.

New feature
– Build a dynamic form (about 6–10 fields) that lets visitors choose their requirements and instantly generates a concise proposal/summary on submission.
– The form should support inline validation, be easy to extend, and match the new Bootstrap styling.

Deliverables
• Re-styled page(s) running on Bootstrap
• Working dynamic form with validation and proposal output
• Commented source files and a short “how to modify” read-me
• Functional prototype deployed to a test URL so I can review before sign-off
