word document and formstack

Hello i'm looking to create an online tool that makes template document by uploading any word document, with custom fields (can be simple keyword in the doc) in them, then generate an online form with the custom field found in the word doc.
Then user can fill in the form. and it will regenerate the original document with the custom field filled.
