Urgent project - Form Design

I want someone who is able to design a PDF form for me, you can use Canva or any other app as long as you can submit the Canva editable link or the editable/source file...attached is the pdf form with the content that I need designed in an elite and professional/modern way. Please note, all content needs to be on there, none can be skipped.

I would prefer if someone can start right away and submit in 3-5 hours or sooner.
