Design Evaluated Interview Template in Word

I need a hand developing an Interview Panel Evaluation template in Word. The layout requires headers and footers as well as page numbers. A special need includes ensuring pasted text maintains consistent formatting.
Ideal candidate should have the following skills:
- Advanced skills in Word.
- Knowledge in formatting fields.
- Page layout aesthetics and arrangements.
The evaluation form should allow to track the technical knowledge of interviewees. I look forward to a solution that would make my interview process more efficient and evaluative.
The format and style (fonts, colors, layout) of the attached document is exactly what I need, but when others use it, there are issues with formatting consistency and how the fields operate. For example, the numbered questions change numbers if pasted text is entered with formatting on it. This document needs a lot of testing to ensure it can be used by all level of users.
It would be especially awesome if the final score could automatically be calculated from the individual scores entered in the score fields above it.
