Modify a flutter mobile app project

Hello. I need a mobile app developer to build a mobile app as an attachment to an existing web application (windows server ). The main app is a survey administration application and the mobile app is expected to ease answering of the survey by the respondents. I can summarize the Modules/functions/pages expected of the mobile app to the following
