Per Forma Financials (BS IS and CFS)

I am looking for help trying to build out a simple BS (5 years) for a start-up. It's a Receivables Purchase business which will be financed by bank debt. Opex will be financed by equity. I have five years of revenue assumptions, SG&A and COGS (bank debt costs and insurance premium fees). I don;t think this would take more than an hour or two for someone experienced. Thank you
