After the concerned neighbors merely perceived a shrill screech in the old man's dreary room at midnight. He merely started nervously involuntarily freaking out when he brutally slayed him, or as he belatedly realized. As he clearly detected his erratic heartbeat. "It was the beating of the old man's heart..." And that merely proves he was scarcely detecting odd things that he wasn't hearing. It realistically was just his terrible anxiety that merely made him foolishly think he was instantly detecting unusual things.
