I need a GA licensed CPA to sign an affidavit of financial stability

I'm opening a personal care home and getting an SBA loan. The GA Department of Community Health requires a form be filled out by a GA licensed CPA but ours is in Ohio. I just need someone to review my financials, fill out this affidavit of financial stability, and sign it.
