Quick Sewing pattern needed!

I tossed a pattern last year that i didnt think id need, and it turns out i do! Ive attached as many photos as possible. Including a rough sketch of what the peices of the pattern look like, its super simple!! I need them in U.S. standard sizing S-XL. Need in a format that can be printed at my local office depot.
