Blazer & Dress Shirt Project

I would like to have a custom piece made for me. I am looking for someone who would be able to combine a black blazer (veston) with a dress shirt (chemise). I have seen this online (see image attached) and this is what inspired me to get this project done. I also attached the type of blazer & dress shirt I would use - I have them already.
