Consent order - checking forms

I am looking for advice/help in my divorce - namely the filing of a consent order. I have been granted a decree nisi and my wife and I are agreeing on how everything is to be split, with nothing contested. We are not claiming anything against each other's pensions etc, just a transfer of equity so that I will sign a quit deed on the shared marital home.

I do have a draft consent order, done using a template from the law depot website - where I'm struggling is how to complete the D81 and A_0422 documents that I have to file.

I was wondering whether you provide a service that would check these forms for me to make sure if I've filled them in correctly, or even draft the consent order if you believe that would be preferable, and the associated costs of each options.
