I work for an event center they have an event log with unnecessary parts to it I would like it to be edit friendly where employees can edit on the computer at work I would also like the logo attached to it

I would like to add where employees can check mark their employee number to see who filled it out the paper work instead of that set up count that people find unnecessary keep the signature part as well.
