uploading excel sheet to get info

i want to have a website :
1- design the home page , ideally 1 page website
2-allow store owners ( pharmacies) to upload csv file i attached 2 , bec most pharmacies use 2 software for prescriptions and patients handling , FILLWARE & KROLL , so i attached a file sample of each .
the store owner can purchase a report for certain price ( we have to activate payment like pay pal ) and after purchase , they can upload their file and our system will generate for them a map .
i attached a sample map , the map will show geo location of the clients of that pharmacy, and will also show bigger markers for people who spend $500 or more and other color patients who spend $1000 or more .
so end result , when the store owner looks at the map we generate for them , it shows where ALL their clients are and the biggest clients in theri pharmacy .
3- some clients may be find it hard , so we can have the ability to upload their file directly and we send them a screenshot of the map .
we have to give the option after they purchase or if their upload does not work to send us the file .
4- you can see that both reports have almost the same info , but we want to show is mostly the the address by markers and when we hover over the marker shows the name of the patient and in a legend we can see the amount spend .
