spreadsheet expert - creating a responsive forms for a sports event

I want to create a couple of forms and a responsive spreadsheet for a cycling event.

Form 1
- individual enters name and weight


Form 2
Individual is reporting against performance in a specific activity
Individuals may complete this form for the same event multiple times

Choices are
- what is your name (from the list generated in form 1)
- what event did you do (choice of 6s, 30s, or 1km)
- Then one of 3 questions (if 6s - What was your average power?, if 30s - What was your average power?, if 1km - What was your time?)


I want the result to be in a spreadsheet with 2 sheets

The first sheet -where each row has the following columns

Name
Event
Average power
Duration
Weight
Average power / weight

The second sheet should be responsive and have 4 sections
The sheet should be readable in a landscape format, for use on a large monitor


Section 1 - 6s competition
5 rows of highest power/ weight achieved, sorted from 1st to 5th descending
Each name should appear a maximum of 1 time


Section 2 - 30s competition
5 rows of highest power/ weight achieved, sorted from 1st to 5th descending
Each name should appear a maximum of 1 time

Section 3 - 1km competition
5 rows of the shortest time
Each name can only appear once

Section 4
A table with 4 columns
Name - the name entered at the start
6s - the highest average power/ weight entered against this category for that individual - if no entry, is blank
30s - the highest average power/weight entered by for that individual - if no entry, is blank
1k - the shortest time entered for that indivdual, if no entry blank
