looking to convert a rich pdf to a Word document or spreadsheet

I have a pdf file (attached) I am looking to convert to a spreadhsheet or word document but must keep the rich graphics feel. Please provide bids , time to complete and past examples.
when creating in either word or spreadsheet, only the last column need be editable so the other fields in the table can be protected. I imagine it will not take more than 2-5 hours for someone good at Word or Excel.
