I and excel expert in Pivot tables, formula and macros

Attached is a spread sheet containing two tabs. The first is a sample of our system raw output shown for just one candidate (usually 800+ each month). The second document is how we want to convert the raw data in the CVS file into a excel document each month for our billing report (again example of one candidate) but usually 800+ each month. We need to find a way to drop the raw data we run from a CVS file each week or month into a excel document with the format provide so the data can be converted each month to the excel report format we need. It is not just a single excel document but a template we can use continuously once built. The original output comes out in CVS file and is likely to contain 15,000+ rows of data maybe 20-30 lines per candidate which need to converted into excel by our team. We run the candidate CVS data in our system weekly and monthly so the expert need to be able to convert this into excel in the format stated we not sure if Pivot tables and formulas work best of advance macros and formulas. I need the work done end of September.
