Re-create excel spreadsheet

I have an excel spreadsheet, but it does not look good. I would like the sheet re-vamped and re-created to display the information better. I am not sure if there is a better way to organize the information at the top or down the sides of the rows. The second tab (criteria template example) is the explanation for others on how to use the spreadsheet an example tab. They rest of the tabs follow as they will be filled out. I am looking for a good working template that I can use to add information, that looks good and is clear. Each tab, I would like the same theme and to look professional. Thank you!
