construct dataset from analog report

I have a report from 1968 where a federal agency provides data for about 3100 counties in the US. These data are handwritten (including commas in the data e.g., 1,200,000). I need a selected set of columns from these handwritten accounts to be converted into digital data that can be manipulated in Excel, statistical programs etc. There are 16 columns of data along with a column identifying the county. I will need the name of the county and the data in columns 8 and 9 as well as columns 15 and 16. If converting all columns is more efficient that is quite acceptable.
For complete transparency this to support a scholarly project--I am an independent scholar who holds a Ph.D.
