Weekly Restaurant Payroll Excel Development

I'm in need of an experienced Excel programmer to design a weekly restaurant payroll spreadsheet for my business, Beach Diner.
The following components are what I require. The Spreadsheet should have a title, Beach Diner Payroll. It should have a spot to place start and end date of payroll. I want a spreadsheet reading categorically by column from left to right. I need the columns to add and subtract totals:
Employee-Title-CurrentPay-(O/T)Rate-TotalHours-(Reg)Hours-(OT)Hours-Pay-Tips-3%Withheld-GrossPay
The first column is the name of the employee. I need forty rows for forty potential employees. The second column is their job title. The third is their current pay. The fourth is their overtime rate of pay, The fifth is total hours worked. The sixth is regular hours. The seventh is overtime hours. The eighth is total pay to be figured by multiplying proper rate of pay by hours worked. The ninth is tips earned (add column to total pay). The tenth is tips withheld (3% of server's total sales, subtract from total pay). The last is Gross Pay.
