VBA Form to PDF and Email as Attachement

I'm in need of a VBA expert to develop a form. The purpose of this form is to submit a report, collecting more than 10 fields of data.
Key requirements:
- Develop a form using VBA that includes more than 10 fields
- The form should be able to create a PDF of the filled form
- Include a button on the form that creates an email with the filled document
Ideal skills and experience:
- Proficient in VBA
- Experience in designing and developing forms
- Familiarity with PDF creation
- Previous experience with form-to-email functionality would be a plus.
