Transcribe PDF to CSV listing orchestral musicians

I'm attaching a scanned PDF listing all the instrumental musicians (approximately 500) who played in the Cleveland orchestra over a 82 year period. I'd like to convert it to a table (csv, Excel, GSheet) giving each musician's instrument, title (if any), first name, last name, and their first and last year in the orchestra, possibly different for each instrument and title. I'm filled in the first few rows or the table so you can see what I mean. I just need the instrumental musicians, not the librarians, personnel managers, or conductors at the end.

Note that this is not easy to automate since the original PDF is not in tabular form. I would like to hear how you will catch and correct errors, for example by having two people enter the data independently. Thanks for your bid!
