Task Completion Checklist with Weighted Metrics

I need a task completion tracking checklist created in both Word and Excel. The checklist should have multi-option checkboxes and include a weighted average for each question. The final output should indicate whether "more engagement is needed" or not.

I need to create a checklist in xls ans word document, that will look professional. Someone good with macros and can create or formulize it basically the requirements are
1. Create a template checklist of 10 qustions
2. Each queation will have 3 resoonses
3. Each response, respondant will have to click a check box than have an excel sheet sort of drop down
4. Each qs will have a weighted grade
5. Once all questions are answered and graded, respondant and everyone will see if further evaluation is needed or engagement with other group. I will give those average.
