Starting Balance Sheet & 12 month Pro-forma Income Statement for a Start-up Business

I have a forecast spreadsheet but need that data put into financial the reporting documents needed for investors and banks. I'd like to know if any other documents may/are needed and/or recommended like Price Earnings Ratio, Trailing Twelve Month, Forward Earnings, Book Value of Shares, Market Capitalization, Rolling Outlook, Earnings Per Share, Annual Yield, Company Value, Return on Assets, Return on Equity.
