Simple Google Map Plotting location

I need a Google map that plots markers based on my private google spreadsheet. I have an example and its simple.

I have an google excel spreadsheet, where i upload Name, District/Precint, Address, Main Category (ex Supermarkets, Barbers) etc, Submitted by

- This will be my master list and only i can update this that will be reflected live

I will have another list where users can provide me with new information accordingly and i can upload that to the live sheet.

It really basic and simple. Need this done asap
