Restaurant Reserve Sign Template

I need a template to create reserve signs for restaurant attendees.

Requirements:
- Individual cards per A4 sheet
- 4 reserve signs per A4 sheet
- Information on each reserve sign: Attendee names, Reservation time, Assigned area
- include logo
- Utilize my existing design
