Research of IPAs

I need someone to find or create me a list of all IPAs which means: “independent physician associations”. Basically these are groups of physicians that are independently and are not employees by a hospital system; rather they join an IPA organization that helps them remain independent. I need lists for all of Florida and all of Texas. IPA NAME AND ADDRESS in an excel spreadsheet.
