Quick Conversion of Emails to CSV

I need someone who can diligently take 138 email addresses, currently sitting on a Word document, and copy-paste them into a CSV file. Here's the scope of the task:
- The emails should be on a new line, each occupying its distinct row.
- Alongside the emails, I would like to have a 'Name' field. The corresponding name to each email address should be accurately filled in this field.
- The resultant file should be compatible with Mailchimp.
- Given the urgency of the task, I need this done ASAP.
