Programming an Excel macro to import XML data into an Excel Workbook

What I am looking for is assistance to create a macro that will do the following in an Excel workbook:
1. Ask the user to locate the XML file
2. Import the data from the XML file and copy it into the correct fields in a form in the workbook.
3. Be assigned to a button so the user can do this at the click of a button.
The XML file is generated from an online form on our website. The output from the form is received either as an XML or pdf file. Both are attached as an example. We then want to import the data into spreadsheet in order to process it further but the data needs to go into specific fields, with the same field names that are in the Excel spreadsheet on the "Application Information" tab and seen in the pdf file.
