Populate answers in google forms with google Sheets

I am looking for someone to assist me with populating answers in a Google Form using Google Sheets.

Type of Google Form: Survey form

Layout or design: I have a specific layout or design in mind for the Google Form.

Google Sheet: I do not have a Google Sheet with the data yet, but I am able to prepare it myself.
