Patient Records Disease Incidence Analysis

I have several Excel workbooks containing anonymised patient records, and I need a clear, reproducible analysis that shows disease incidence rates across the dataset. The files are already in Excel, so you can work directly with pivot tables, Power Query, or any other native Excel features you prefer—no conversion necessary.
Scope of work
• Clean and structure the existing sheets where required (dates, duplicate IDs, missing values).
• Calculate overall and subgroup-specific incidence rates; age, gender, and any other readily available demographic fields should be included if present.
• Summarise findings in an easy-to-read dashboard: tables, charts, and a short narrative on methods used so I can replicate future runs.
Deliverables
1. A final Excel file with all formulas or pivot tables intact.
2. A one-page note (Word or PDF) briefly outlining steps taken, assumptions, and how to refresh the analysis when new patient records are added.
Acceptance criteria
– Incidence rates automatically update when new rows are appended.
– No external add-ins required; everything must run in standard Excel 2016+.
– All computations transparent: I should see the formulas or M-code used.
