Patient Record Statistical Summary

I have a dataset of patient records that needs a clear, well-organised statistical summary in Excel. The core of the job is to calculate mean and median values for the key numerical fields and map out frequency distributions so trends in demographics, diagnoses, or treatment outcomes are immediately visible.
I will supply the raw spreadsheet—columns are already labelled—along with a short data dictionary. Your task is to:
• build the formulas or PivotTables that return the mean, median and frequency counts
• present the results on a clean, separate summary sheet with easy-to-read tables and, where helpful, simple charts
• keep every calculation fully transparent (no hidden cells or hard-coded numbers) so I can trace or update results later
