Need an excel dashboard created

I am a Change manager and I am capturing change impacts on a project. I want to be able to report on specifics of the impacts, such as number of changes, intensity, perception, type of change by workstream, stakeholder group and functional area.

Slightly complicated by the fact that I want to be able to demonstrate that a single change may have multiple impacts depending on # of stakeholder groups impacted.
