Need Email Address List Converted from TXT to Formatted CSV or XLS FIle

I am looking for a freelancer who can convert my email address list from a TXT file to a formatted XLS file with 2 columns entitled 'Name' and 'Email'.
I am providing a TXT file with about 700 email addresses. There are names for most of the email addresses. The list you create must include the name and email within the same row among two columns. I have attached an example of how the final XLS/CSV file should look upon conversion.
Format:
- The converted file should be in XLS format.
Specific Data Fields:
